India's space agency Isro faced two consecutive PSLV rocket failures in 2025 and 2026, delaying NavIC satellite launches, private missions, and the Small Satellite Launch Vehicle program. An expert committee headed by former scientific adviser Prof. Vijayraghavan identified third-stage anomalies as the cause. The committee will submit findings by June 2026. Isro chairman stated trajectory deviations occurred during the third stage. Officials expect launches to resume within months after resolving identified issues.
